Description
A carver is the double-ended hand instrument used to create cusp and groove anatomy on the occlusal surface of a restoration. Clinically, once an amalgam restoration has been condensed into the cavity, excess material is removed before the amalgam sets and the cusp tips and fissure pattern are shaped with the carver to match the neighbouring tooth anatomy. This step is not merely cosmetic: an occlusal surface that has not been carved correctly creates a premature contact, which can fracture the restoration margin and leave the patient with an uncomfortable bite. The instrument is also used to make the restoration margin continuous with the tooth surface, since material left standing at the margin encourages plaque accumulation and secondary caries. In the dental laboratory the carver is used to build the occlusal surface in wax modelling and to remove surplus wax. Keeping the blade sharp matters: a dull carver drags the material instead of cutting it and leaves marks on the surface. The instrument is stainless steel and can be sterilised in an autoclave.
